Case Manager for Bundaberg, QLD

We at Conexson are working with one of our key clients to recruit for a Case Manager.
Do you enjoy assisting people in finding the best care options to meet their mental, physical, and emotional health?
We are looking for capable case managers who are conscientious, can effectively communicate with clients, and can lead a team of professionals to achieve goals and milestones.

Your role will include but is not limited to:

  • Assess client needs and goals.
  • Plan a comprehensive care plan based on clients' needs.
  • Ensure effective delivery of high-quality care and service.
  • Work collaboratively with other team members, organizations, support workers and service agencies to ensure the best outcome for clients.
  • Manage feedback and complaints and undertake continuous improvement.
  • Conduct and assist with internal audits.
  • Ensure confidentiality.
  • Respond to problematic situations promptly and develop radical solutions to discuss with Coordinators and Managers.

Requirements:

  • Qualifications in Community Services, Human Services, Health Sector Management, Aged Care and/or Clinical background (minimum Cert III) and a minimum of 3 years experience in the industry.
  • Demonstrated understanding of the Aged Care reform, Aged Care standards, the sector, and people who are aging in the community.
  • Experienced in case management preferably within the home and community care sector.
  • Experience in service development and driving continuous quality improvement and practice and service excellence aligning to Aged Care Quality Standards.
  • Is relatable and engaging with highly developed interpersonal, communication skills, and emotional intelligence with the proven ability to foster relationships and connect with internal and external stakeholders.

Special Work Requirements:

  • Driver's License and National Criminal History Check
  • Prescribed doses of COVID and Influenza vaccinations.
