The Way Back Support Service delivers community based, psychosocial support to people at risk of suicide, following a suicide attempt or during suicide crisis. Our mission is to foster an independent and positive future for individuals dealing with mental health issues and disabilities.

The opportunity:
The position of Support Coordinator is currently open within our The Way Back Support Service in Maroochydore. In this role you will be connecting with clients and tailoring a program for up to three months after discharge from hospital. This is a full time maximum term contract for 6 months with the possibility of extension.

Throughout the 12-week program, you will collaborate with the client to address barriers they have identified as contributing to their stressors. Additionally, you will strive to empower the client by involving them in their recovery process and enhancing their awareness of available supports once they complete the program. This program is built on personal connection and integration with community services.

Being in this role can make a world of difference to someone going through their toughest moments. Witnessing clients take positive strides forward in their lives can bring immense satisfaction.

Responsibilities include:
• Foster and sustain connections, utilising a psychosocial approach to assist individuals in need.
• Be actively responsible & participate in the collection of data for the purposes of monitoring, reporting, and evaluating.
• Build and develop strong relationships with clients, services providers, and health care professionals.
• Liaise with hospitals, general practitioners, and health services.

About you:
You are passionate about fostering engagement with clients, service providers and the community and are skilled in the ability to develop individual safety plans. We are seeking welcoming, professional, and committed individuals, with the following attributes:
• Possess qualifications in the Human Services field, such as Social Work, Nursing, or Occupational Therapy.
• Demonstrate experience in mental health services and/or disability.
• Exhibit a dedicated and passionate commitment to teamwork for positive change in the lives of individuals with severe mental illness.
• Showcase the ability to work both independently and collaboratively within a team, ensuring effective delivery of The Way Back service, with exceptional organizational skills.
• Exhibit a willingness to support complex clients dealing with a range of issues.
