Primary purpose of the roleThe Doorways Outreach Caseworker provides support primarily for those accommodated in the Minderoo Foundation temporary accommodation pods as a result of the WA Wooroloo fires. The program is an assertive outreach service operating within the area of the Wooroloo fires and peripheral areas engaging with individuals and/or families, assessing their immediate needs and exploring accommodation options to transition out to the temporary pods and into sustainable accommodation.
Key ResponsibilitiesPlan, initiate and deliver individual case plans to meet the range of needs facing participating clients
Ensure the appropriate assessment of clients
Provide case management for each individual/ family entering the programme and record precise case management notes and details
Provide clients with appropriate information and resources to assist them in making informed decisions to transition from the temporary pods to stable accommodation.
Write reports and letters, complete surveys and ensure all client files are kept in an orderly fashion
Assist in the development of community/individual living skills and educational groups where required
Research and analyse community need to assist with successful service outcomes and completed in a timely fashion of all necessary documentation
Maintain a statistical database
Qualifications in social welfare or related discipline and/or (up to 5 years) previous practical experience in the delivery of outreach case management to clients in the need of accommodation assistance and life skills training
Ability to effectively communicate with all levels of people from a range of diverse backgrounds including the general public TSA, government and non-government departments and the public
Demonstrated experience to work as a team member in a diverse community service setting
Demonstrated experience in the assessment of clients’ needs for medium to long term accommodation and associated issues
Current “C” class driver’s license
