The Team Leader Service Delivery's primary objective is to ensure that Council:

  • Provides opportunities for eligible residents to maintain independence and a lifestyle in their place of residence and local community.
  • Can effectively deliver Commonwealth and State Government funded community care service programs, for eligible people within Council’s service district by appropriately trained service providers. 
  • Coordinates a team of Community Care Workers in the delivery of services.
  • Oversees the administrative duties in the delivery of services.

Key responsibilities include:

  • Manage the assessment and re-assessment, and care plans, of eligible people for all Commonwealth and State Government funded community care services.
  • Oversee the delivery of all community care services by community care staff.
  • Ensure that a sufficient number of volunteers are available for the delivery of meals.
  • Manage the delivery of Commonwealth and State Government funded community care services, including the allocation of resources and tasks and the management and development of staff. 
  • Provide support and direction to internal staff, field staff (community care workers) and volunteers.
  • Manage community care worker training requirements and programs.
  • Maintain administrative systems, including computer records, debtors, payroll, service provision statistics, MDS, QR and HACC service reports.

About You:

To support your success in this role you will have:

  • Demonstrated knowledge and experience of the Commonwealth Home Support Program, State-Funded HACCPYP program, Veterans Home Care and National Disability Insurance Scheme program.
  • Demonstrated ability to recruit, supervise and monitor a team of Community Care Workers and Volunteers.
  • Highly developed written and verbal communication skills dealing with a range of internal and external stakeholders.
  • A current Victorian Driver’s Licence.
  • Current First Aid Certificate, or willingness to obtain.
  • Police Check Clearance and current Working with Children Check
