  • Fixed Term position until 30th June 2025  
  • Full time, however a secondment could be considered, as well as part time hours

The Department

This busy and dynamic interdisciplinary team conduct independent comprehensive assessment to aged persons and some younger persons with a disability; and determines the eligibility for a range of Australian government subsidised services, including residential aged care, residential respite, home care packages and flexible care.

The Role

You will be responsible for the comprehensive assessment of a person’s physical, medical, psychological, environmental and social needs to determine the care and services required. You require skills in assessment and the ability to work autonomously. Assessments are mostly undertaken in the community setting but may be in a residential aged care setting or a hospital.

Experience and Qualifications Required

Allied Health

  • Degree or equivalent in a relevant Allied Health discipline
  • AHPRA (Australian Health Practitioners Regulation Agency) registered or eligible for professional membership if a self-regulated Allied Health profession e.g.: 
    • Eligible for membership of the Australian Association of Social Workers and agreeable to provide an annual Statutory Declaration stating that the credentialing requirements have been met for Social Workers at Alfred Health   

Nursing

  • Degree in Bachelor of Nursing or equivalent
  • Registered with the Nursing and Midwifery Board of Australia via AHPRA
  • Must have advanced assessment skills
  • Community and aged care experience
  • Current Victorian Drivers licence is essential
