We are looking for a proactive and energetic individual to work in our Shared Living division based in Concord West. Our new teammate will be providing support when shifted for the all-important sleep over shift.
About the Role
As a Support Worker, you will play an essential role in supporting and empowering people with disabilities in their Supported Independent Living (SIL) accommodation. We are a friendly and supportive team that thrives on not only teamwork but your proactivity and professionalism.

Key focus
Supporting clients in their group accommodation during active hours. You will also be on-site overnight and be expected to support during the inactive / sleep break when necessary
Support residents to ensure household tasks meet the organisational requirements that their home is clean, tidy and sanitized.  
Daily routines: assist and support clients with their routines which may include personal care, medication administration, special menu planning, laundry, person lead activities in their homes and out in the community
Maintenance of  accurate progress notes / incident reporting to meet organisational and NDIS requirements

About you
You are an adept team player and have experience supporting people with complex behaviours and health needs
Demonstrated experience following client focused plans including behaviour support plans (BSP), medication management plans and other health related plans
High levels of enthusiasm, initiative and positivity while maintaining Sunnyfield values:  Respect, Trust, Honesty and Innovation.
Your are able to accept a roster consisting of sleepover shifts and how they underscore service delivery
Minimum Certificate III in Disability, Individual Support or Community Services (or similar)
