About the role

This Level 3 Senior Social Worker fixed-term full-time opportunity until June 2025 is based at Braeside Hospital, Prairiewood. It is open to Level 1, 2 or 3 Social Workers and is to work within the Mission and Mission in Action of HammondCare to ensure a high standard of Social Work service is provided to patients of the Hospital, along with appropriate support and advice to their carers. The Senior Social Worker will participate as a member of the multi-disciplinary team to ensure a patient-centred, evidence-based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare and the Australian Association of Social Workers (AASW).


The essence of this role

Provision of effective and efficient social work service to patients, including appropriate support and advice to their families and carers Participate as a member of the multi-disciplinary team in assessing patients’ level of functional and care needs, setting care goals and providing appropriate treatment. Participate as a member of the HammondCare team in the application of the organisation’s mission and values to the pursuit of its objectives in a client-centred, continuous quality improvement work environment


About you

Commitment to align and engage with HammondCare’s Mission, Motivation and Mission in Action A tertiary qualification in Social Work, providing eligibility and membership to the Australian Association of Social Workers. Qualifications obtained overseas must provide AASW Overseas Assessment number.  Advanced specialist knowledge and clinical skills in delivering Social Work services to Rehabilitation, Aged Care Psychiatry and/or Palliative Care setting.  Superior interpersonal, consultation and reasoning skills necessary to ensure high clinical practice standards and build strong team practice, using a variety of media – oral, written and computer.  Demonstrated use of advanced level professional judgement and ability to lead and independently problem-solve set priorities in complex situations.  Demonstrated advanced-level skills and experience in interacting constructively and collaboratively with the internal multidisciplinary team and external service providers.  Demonstrated skills and experience in monitoring progress, performance and providing clinical supervision and clinical leadership.  Experience in quality improvement initiatives aimed at enhancing service delivery, including planning, development, implementation, evaluation and professional education and training.  An unencumbered Driver’s License and willingness to travel between HammondCare’s sites and services.
