We are a not-for-profit, values-based organisation that has been offering residential, community, and in-home aged care to older people living in the municipality of Manningham and surrounding LGAs for over 30 years.

Are you ready to take on an exciting new role as part of our Community Team? We are currently looking for a dedicated and detail-oriented individual to join us as an Admissions/Intake Coordinator. In this role, you will play a vital part in the smooth administration of new client intakes for our Commonwealth Home Support Program (CHSP) funding stream. Your responsibilities will include managing the process of enrolling new clients and ensuring that they receive the appropriate care and support they need.

About the role

  • Manage new client inbound and outbound calls; promptly respond to inquiries and referrals via phone, email, and online.
  • Planning and coordinating appointments/home presentations developed within the admissions program.
  • Provide quality customer service to potential new clients, aiming to meet these clients' needs efficiently and effectively and provide the best possible care and support service.
  • Entering client information into the client management system.
  • Completing an initial assessment and care plan to inform the case managers.
  • Managing vital database record keeping and following up on clients and case managers to ensure we deliver services in line with our service.
  • Working alongside different departments to respond to client referrals and inquiries.
  • Identify appropriate internal or external programs and services and provide referral pathways for clients and their families.
  • Develop and maintain positive stakeholder relationships and engagement and build networks to promote and market Mannacare’s brand.

About you

  • Experience within an aged care environment
  • Knowledge of the Community Home Support Program (CHSP) funding stream.
  • Demonstrated understanding of sales
  • Experience with data entry, referral management, portals, and other referral media
  • Strong Microsoft Office skills with the ability to learn and adapt to a variety of client management systems
  • Ability to communicate effectively with a range of people, including older people, staff, families, and particularly concerning the care of consumers
  • Outstanding time management skills with an established caseload
  • Self-motivated with the ability to work unsupervised and as part of a team
  • Current NDIS Worker Screening Check
  • Current Driver’s License and comprehensively insured and registered vehicle
