About the Role

The Nurse Coordinator is a key member of the nursing leadership team. Nurse Coordinators utilise and share specialist knowledge, skills and the essence of nursing to ensure excellence in care for patients supported by Wallaby Ward (HITH). The Nurse Coordinator will work to improve access, flow and the transfer of patients from inpatient wards, outpatient departments and the emergency department to Wallaby Ward. This position will oversee and coordinate patients under the General Medical, Respiratory, Neurology and Gastroenterology teams. This is a permanent 0.6 FTE, part time Monday to Wednesday role.

What you’ll achieve:

• Manage clinical care to optimise patient outcomes and flow through the ward/unit
• Works collaboratively with all clinicians to provide appropriate and timely management, ensuring a structured and efficient plan of care
• Maintain existing protocols and develop new protocols as needed to provide excellence in care
• Provide education and training to the ward

Classification for this position is HITH/PAC Nurse Level 2 (YW15). 

Your skills and experience

• Extensive experience in acute paediatric nursing
• Demonstrated commitment to ongoing personal and professional development, as evidenced in a Continuing Professional Development (CPD) record/Professional Practice Portfolio (PPP)
• Experience in care co-ordination

About Wallaby Ward

The Hospital-in-the-Home program (Wallaby Ward) allows children to be cared for at home by The Royal Children’s Hospital (RCH) medical, nursing and allied health staff.

Wallaby is a 61 bed ward, in which medically stable children requiring daily/twice daily acute care interventions can complete their admission at home. Whilst at home, children are visited and cared for by a team of qualified RCH nurses, medical and allied health staff who communicate regularly with the child's primary treating medical unit.
