Key Responsibilities may include:

• Collecting information from clients and planning partners, both over the phone and in person, to support high-quality planning and evidence-based decision-making.
• Applying a variety of policies, practice guidelines, and legislation to make informed funding decisions that enhance participant plans and complement informal, community, and mainstream supports.
• Collaborating closely with clients and their representatives to identify current and future support needs, including informal, mainstream, and community support options, to achieve plan outcomes.
• Coordinating with key stakeholders, to effectively deliver services to clients.
• Addressing and resolving issues and complaints related to plans.
