Specialist Dementia Care Manager

We are seeking a passionate Registered Nurse with management and DEMENTIA care experience to join our new 8-bed transitional Specialist Dementia Care Unit (Waratah). This position will also support our existing Memory Support unit; Merton (11 beds) as well as be a resource and specialist in dementia for the home to support the management and oversight of the use of psychotropic medications.
This is a rewarding opportunity where you will lead and manage the clinical care requirements of Waratah unit, with overarching support from the Facility Manager. You will demonstrate sound clinical reasoning, leadership, and education skills to support and model contemporary dementia care practice to reduce the impact of responsive behaviours within the unit.
You will have the opportunity to work with a collaborative team of health professionals including a specialised Dementia care in-reach team of practitioners, along with employer's own specialist dementia care team. Case Management skills and the ability to work closely with in-reach specialist and allied health providers are key skills to the role's success.

Responsibilities
• Prepare reports and collate information for unit reporting requirements.
• Conduct pre-entry assessments for resident suitability to the unit.
• Maintain the unit's documentation, registers, and practice to maintain compliance with ACQSC standards.
• Prepare individualised behaviour management support plans in collaboration with the unit team members and the in-reach specialists.
• Lead staff through demonstration of clinically relevant engagement and psychosocial support strategies for the person living with dementia.
• Understand and develop a case management approach to care that focuses on behaviour support initiatives, care planning, quality of life goals, and successful transition planning that is person-centred.
• Chair resident-focused unit meetings with Glenview management, and weekly in-reach, and specialist care teams.
• Model evidence-based dementia care skills and assessment techniques to other employees to build team knowledge and practice.
• Plan and educate staff on engagement techniques to ensure resident quality of life and psychosocial care objectives are met or exceeded.
• Manage transition in and out of the unit with the involvement of the specialist in reach team as contributors.
• Lead a small workforce in the SDCU and Merton MSU.
Skills
• Demonstrated expertise in dementia care and support within residential, acute, or community settings.
• A leadership approach that is positive, and a calm and considered disposition.
• Demonstrated management skills and the ability to lead a small team.
• Proficiency in computer literacy, navigation, and use.
• A passion and ability to deliver person-centered quality dementia care that is responsive and reactive to variable influences, e.g., environmental, and clinical.
• Effective time management and the ability to work as a team to problem solve and guide decision making.
• Clear and objective writing skills, with clear verbal communication skills
• Empathy and patience with resident-focused values
• Provision of clinical care in a non-clinical setting to maintain a home-like environment.
• A commitment to building relationships with residents, representatives, in-reach consultants, colleagues, Glenview's own specialist dementia care consultant, and team.The ability to work collaboratively with a multidisciplinary team.
Qualifications Required:
• Registered Nurse, Level 3 with a valid AHPRA registration - without practice limitations.
Highly desirable:
• Graduate certificate, Diploma or Bachelor in Dementia Care
• Postgraduate qualifications in Dementia Care or Gerontology (certificate or above)
• UTAS Dementia MOOC /or Aust Govt Dementia Training Program
• Experience working in an aged care, dementia care environments in any healthcare setting.
