The Aged Care Clinical Nurse Specialist 2 (CNS2) role provides specialist clinical practice undertaking and assisting in acute assessments of geriatric patients in residential Aged Care facilities. The role provides education, advice and supports to other health professionals at residential aged care homes in the management of Geriatric patients. This position works in collaboration with the aged care CNC;s, Nurse practitioners, transitional nurse practitioners, registered nurses and Geriatricians. The ACOS CNS2 role will provide clinical and professional leadership over the Aged Care outreach service and its key stakeholders.
