**Description**:
We are currently recruiting for a **Registered Nurse/ Enrolled Nurse - Care Coordinator**. Our service include, aged and disability care, transitional care, post-operative care and palliative care. Right at Home is a leading home care provider in the USA and is now launching its operations in Australia.

This role is a full-time management position reporting to the Care Manager/Managing Director. The job would involve coordinating and allocating your care team and ensuring that client needs are met. You will also be responsible for assessment of HCP and private clients.

**Qualifications**:

- Current NSW Driver's Licence
- First Aid and CPR Certificate
- Police Check, or willingness to obtain.

**Preferred Skills**:

- Essential Criteria:_
- Endorsed Enrolled Nurse, Registered Nurse, allied health or equivalent experience in Aged Care
- Sound understanding of the principles of My Aged Care, Aged Care Reforms and Consumer Directed Care
- Comprehensive Assessment and Case Management experience
- Experience in provision of care and services to people from culturally & linguistically diverse backgrounds
- Sound knowledge of aged care services and networks in the centre of the Padstow St George area
- Commitment to work within RAH Mission, Vision and Values
- Capacity to take initiative and to be innovative
- Highly developed organisational, administrative and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines.
- Current NSW Drivers Licence
