CBCHS is a for-cause, not-for-profit organisation providing a wide range of allied health, dental, child development, disability and aged care services to more than 15,000 clients every year in and around the City of Kingston. Many of our clients are differently abled, socially isolated, financially disadvantaged or exposed to other socio-economic vulnerabilities.

CBCHS Adult Community Options are run from multiple sites across the south eastern suburbs, two unique Parkdale sites, Edithvale, Clayton South and Cheltenham. We provide support when and where you need it - at our four unique day support centres, in the community, or in your home.

**About the Role**

**Central Bayside Community Health Services (CBCHS**) - Adult Community Options team has an exciting opportunities for compassionate and motivated Client Support Professionals.

We are looking for a vibrant and passionate Client Support Professional to work collectively and enhance our team on** Full Time and Part Time Permanent** basis. With flexible hours to suit you between 8.30am and 3.30pm, Monday to Friday with **no late nights or weekend work required**. We support people with a range of abilities, from independent, high and complex support needs.

We offer a variety of choice with interesting group programs. Our programs a Therapy and wellness based activities. Some of our favourite activities include sensory and well-being activities, Yoga, and gentle exercise. We are also guided by allied health professional plans to support occupational therapy, physiotherapy, sensory and communication based activities. We take time to understand their goals and aspirations, and pro-actively develop positive support strategies to enhance their quality of life.

Our professional support team provides high a standard of support with self-care and personal hygiene, hoist and transfer assistance, health support including medication, PEG feeding, administering medication, epilepsy and diabetes management.

Our team has a focus in delivering support for local community, making a real difference to our clients lives.

Delivering high quality outcomes for clients. Provides support to a range of people with a disability in all activities, including tasks that involve manual handling and personal hygiene.

Focus on wellness based supports, mornings are based around meditation and low stimulating activities. The afternoon includes community and centred based activities. We welcome clients the freedom to be seen and heard.

**About our clients**

**Some of our favourite onsite activities** are generally sensory based & focus on wellbeing of the mind and body that will enhance & develop our 5 senses touch, taste, sight, sound & smell. These activities include yoga, music therapy, gardening, garden walks, cooking/baking, café visits, art therapy & more.

Our participants have high intensity support needs. This includes sensory, mobility, communication and mealtime care & support. The 5 senses are incorporated into our daily activities as these provide our participants with ways to aid communication.

We have a no lift policy and ensure safety is paramount for staff and participants.

**Responsibilities**

Client Support/Program Delivery
- Passionate about delivering high quality outcomes for clients.
- Ability and willingness to provide support to a range of people with a disability in all activities - tell us about your passion or talent in the cover letter i.e Yoga, Art, Dance, Cooking, sporting etc
- Your passion for clients will see you put the client's choice and control first, and ensure you protect, promote and uphold the rights of people with a disability.
- Providing active day-to-day care, support and direction to clients in both 1:1 and/or group programs.
- Providing full support to clients in all centred and community programs (including swimming).
- Individual personal care needs of clients, as required (including full physical assistance with lunches and incontinent clients.)1:1 centre based support to clients to actively participate in activities.
- Actively support clients with Behaviours of Concern, in accordance with their Behaviour Support Plan in consultation with the site manager.
- Liaise with site supervisor about any matters related to clients including the report any incidents immediately they may occur - this may be verbally to the site manager.
- Always supporting the rights of people with a disability, in accordance with Convention on the Rights of Persons with Disabilities.
