This is an exciting opportunity for an experiecned case manager to join a value's led organisation within their Permanency Support Program (PSP).

Reporting to the Permanency Team Leader, the Permanency Case Manager will:

- Provide clients with flexible and responsive therapeutic support, practical advice and psychoeducation.
- Provide therapeutic case management supports to children/young people, families and carers.
- Actively support permanency goals and tasks directed towards preservation, restoration, adoption or guardianship (in the first instance) or foster care (where appropriate).

This role is full time and is a permanent contract. This is an outreach role and so services will be provided in the field - pool cars are provided for all travel.

**About You**
- Tertiary Qualification in Social Work or related field
- Knowledge of best practice support an treatment for children, young people and parents
- Therapeutic Case Management experience
- Ability to adhere to therapeutic plans to help people of all ages and education levels develop interpersonal skills
- Empathy and understanding of professional boundaries
