**The Role**
We are seeking a Social Worker - Specialist Support Coordinator who is flexible, innovative, motivated and passionate about delivering exceptional services and supports to individuals living with disability. We require someone with energy, enthusiasm and initiative who will thrive in an agile and changing environment. The purpose of this role is to provide Support Coordination including Specialist Support Coordination to NDIS participants and assist them to achieve their goals as set out in their NDIS plans whilst focusing on person-centred delivery of a high standard.
**Specific responsibilities for this role include**:
- Excellent case management experience is essential.
- Knowledge and understanding of the risk factors experienced by each participant with high-risk and/or complex needs.
- Ability to proactively engage to ensure that all providers implementing the plan understand and respond to the risk and or complexity of the situation and collaborate with other relevant providers when required.
- Providing relevant reports to NDIS in a timely manner
- Ensure regular stakeholder meetings and follow up duties are completed.
- Ensuring participants budgets are utilised and maximised to meet their needs, further to this maintain participants budgets and communicate these to relevant stakeholders
- Undertaking work in line with the organisations policies, procedures, and values
- Maintaining an up-to-date knowledge of the NDIS systems, practices, all relevant legislation and codes of conduct
- Maintaining an up-to-date knowledge of local services and organisations participants can utilise
- Undertake professional development activities and maintain required professional ethical standards.
- Participate in peer review and supervision and contribute to the quality of case work delivery through team practice reflections.
- Design, contribute to, and participate in, planning/review processes within the Service.
- Cooperate with all health and safety policies and procedures of Well Life Services and take all reasonable care to ensure that your actions or omissions do not impact on the health and safety of yourself or others.
**Requirements**:
- Demonstrated experience as a Specialist Support Coordinator working with NDIS participants
- Demonstrated high level interpersonal, verbal and written communication skills
- Demonstrated ability to work collaboratively and supportively within a work team
- Excellent time management skills and ability to prioritise
- Experience in service development and evaluation including initiating and coordinating quality improvement.
- Proficiency in Careview and Microsoft Sharepoint (Desired)
- Proven desire to actively participate in professional development.
- Demonstrated understanding of the sustainability of service delivery and be competent in reporting to the
Director on monthly reports and KPI's.
