**Support Worker**

**Duties will include**:

- Supporting individuals to reach their goals within their chosen community.
- Working within a flexible roster
- Supporting Clients within their private residence, to achieve independence in all daily activities including home duties and personal care
- Reporting to the Client Coordinator as required and working within a team towards continuous improvement

**You must hold;**
- Current National Police Check / Yellow Card / Blue Card
- If you have been a citizen in another country after the age of 16 you will also need to have a statutory declaration as per the "Police Certificate Guidelines for Aged Care Providers"
- Certificate III or IV in Aged Care or Certificate III in Home and Community Care or Certificate III in Disability
- Current First Aid and CPR certificate
- Current Drivers License with own fully registered and insured vehicle
- **Current Medication Competency (highly desirable)**:

- Community Care experience
- Empathy for and desire to work
- Good verbal and written communication skills
