An exciting opportunity for a Social Worker (Grade 2) to join the Mental Wellbeing team based at our Daylesford Health campus. This position provides services to community clients, residential aged care residents and hospital in-patients, collectively known as consumers. The role is to assist and support consumers, their families and support networks in response to physical, emotional and social factors that negatively impact on the consumer's health and wellbeing.
- The social work role also has a primary and secondary prevention component focused on averting the onset of poor mental and physical health and early intervention by addressing social and environmental determinants of health.**_ Details of Appointment_**
Permanent Partime position working 56 hours per fortnight
**Salary**: Dependent on experience, as per EBA at $43.89 - SC21 (Grade 2 Year 1) to $50.46SC24 (Grade 2 Year 4)Per hour + 10.5% superannuation + access to salary packaging

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30th November 2019. Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
