Now Hiring: In-Office Case Manager (California)Salary Range: $100K to $120K a yearLocation: North Richmond, CaliforniaJob Type: Full-TimeSchedule: Monday to Friday, 9 AM – 5 PMOur California-based behavioral health and social services clinic is seeking dedicated, in-office Case Managers to join our compassionate team.Position Summary:The Case Manager plays a key role in coordinating client services, advocating for needs, and guiding individuals toward healthier, more independent lives.
You will work closely with clients in person, providing assessments, referrals, support, and follow-up in a structured, welcoming office environment.Key Responsibilities:Conduct comprehensive client intakes and needs assessmentsDevelop and implement individualized service and care plansProvide direct referrals and connections to housing, health care, employment, and community resourcesAdvocate on behalf of clients with service providers and agenciesMaintain accurate and timely case documentation using EHR systemsCoordinate with therapists, social workers, and program directors as part of a multidisciplinary teamMonitor client progress and adjust service plans as neededEnsure compliance with privacy regulations and agency policiesBachelor's degree in Social Work, Psychology, Human Services, or related field (required)Master's degree or prior case management experience (preferred)Strong knowledge of community resources and support systems in CaliforniaExcellent communication, time management, and organizational skillsProficient with documentation and case management softwareAbility to work on-site and interact directly with clients in the officeBilingual (English/Spanish or other) is a plus but not requiredCompetitive salary and flexible hoursSupportive, family-like team cultureContinuing education opportunitiesModern, child-friendly therapy spaces and toolsOpportunities for professional growth and mentorshipIf you're a compassionate, organized, and motivated individual ready to help others navigate life's challenges, we want to meet you
