We also support our sister company, Centacare Community Services for disability and aged care requirements.With an established reputation, we have been making a difference in the lives of children and young people since 1949.Xavier provides services for: Children, young and older people living with a disability - with development and/or learning challenges with complex disabilities impacting health and well-being and/or requiring Enteral Feeding scripts and meal-time feeding plans Aged person living at home- Complex health and well-being needs with the risk of malnutrition or feeding difficulties The PositionWe have two new part-time positions available for Dietitians who are team players, self-driven, enjoy a varied caseload and are passionate about what they do.
Positions will be based in Brisbane South and Brisbane North.Dietetics is a a new service offering at Xavier, and we are looking for individuals who enjoy the challenge of developing and growing a new service.
Our clients require nutritional assessments and therapeutic planning to meet their individual nutritional needs.
Services are delivered in the home, in the community, in schools, clinics and workplaces as preferred by the client.
The role will plan and provide services and outcomes that are contemporary and evidence-based including: Provide client nutrition assessment, risk and support/care planning utilising appropriate methods, intervention and monitoring of outcomes considering key disability-specific, paediatric and the aged person issues Prescribe appropriate mealtime plans for clients with risk of malnutrition including dysphagia or other physical conditions.
Includes prescription of appropriate enteral feeding plans for children and adults Determine health priorities for complex clients.
Assess the capacity and capability of clients and/or their carers to follow dietetic recommendations and adjust interventions accordingly Refer and coordinate with specialist/support services available for client groups.
Develop and foster professional working relationships with all stakeholders Integrate service delivery and follow up, coordinate and communicate as required with other members of the multi-disciplinary team Deliver education services to staff, clients and their families as relevant to dietetics and assist with facilitation of disability-related and aged care funding that may impact an individual's access to health and dietetic care Contribute to the development of the allied health business plan and service development including: Promote services and contribute and participate in marketing and business development activities Seek to build and maintain a client base through waiting list and capacity management Contribute and participate proactively to a culture of continuous improvement and learning Maintain and develop knowledge and clinical skills based on the latest evidence-based practice and participate in mentoring/supervision as required About youTo be successful we require: Demonstrated knowledge and skills in providing nutrition therapy, assessment and planning for individuals with complex health and disability needs and the aged person utilising appropriate methods intervention and monitoring Demonstrated ability to consult and use evidence-based clinical reasoning to risk-assess and provide recommendations for meal planning in disability-related residential/respite facilities Demonstrated ability to drive referral growth and coordinate with other relevant specialist/support services Demonstrated ability to organise and manage a caseload within a community care setting and ability to work both independently and collaboratively as required within a multi-disciplinary team Strong person-centred focus with ability to communicate professionally, establish trust, rapport and to maintain confidentiality Sound record keeping and information technology skills Sound understanding of scope of practice and professional boundaries A recognised university degree and related qualification in Dietetics and Nutrition Eligibility for full membership of Dietitians Australia and have the Accredited Practicing Dietitian credential Meet suitability requirements including Blue Card, NDIS Worker Screening Card and National Police Check Australian driver licence and reliable vehicle Appropriate work rights for Australia Commitment to upholding the Xavier Values and to the safeguarding of children and vulnerable people at all times Benefits & Culture Supportive multidisciplinary team FT/PT options and a flexible, family-friendly approach to how we work – ask us for what you need to be your best Hybrid work arrangement - work home or at one of our offices Clinical supervision and mentoring designed to meet your individual training and support needs (up to 50 hours per year) Professional development opportunities including an additional 5 days leave (pro rata) and financial support Fortnightly internal professional development and team meetings to stay connected Generous salary sacrifice options (significantly increase your take home pay).
