ABOUT THE ROLEWe are looking for aCare Partner, Registered Nursewho is a passionate health and aged care professional looking for a new opportunity.
As Care Partner you will be an integral point of contact for workers and clients to provide support for the broader community team.
Join us on a permanent full-time capacity at Right at Home Sunshine Coast, located in Caloundra.To be successful you must hold:Formal tertiary qualification eg.
Bachelor of Nursing or equivalentCurrent AHPRA registration as a Registered Nurse (Mandatory)National criminal history/police checkValid First Aid and CPR certificateValid Blue CardExperience:Knowledge of the aged care industry/issues or Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles and the Aged Care Quality Standards - EssentialPrevious experience in Aged Care or related discipline - DesirableEasily adaptable to new software platformsAs aCare Partneryou will be responsible for:Building strong and meaningful connections with clients and their familyConducting comprehensive client assessments and development of care plans to support our clients to remain within their homeInteracting and providing professional health and clinical care advice and direction to a team of home care workersProfessionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfactionEnsuring all practices are consistent within policy and legislative frameworksMaintaining up to date client records and informationABOUT USRight at Home is a leading provider of in-home care and assistance, committed to improving the quality of life for our clients we serve.
