Right at Home Brisbane North And South, a leading provider of care services in the Aged Care and Disability Care Sector, is currently searching for an enthusiastic and dedicated Full-Time Care Coordinator to join our dynamic team.
This is a fantastic opportunity for individuals looking to advance their career and make a meaningful impact in the lives of our clients.
Responsibilities:
Build strong relationships with clients and care workers, ensuring seamless communication and exceptional service.
Manage a caseload of up to 35 clients living with various disabilities, coordinating their care plans, budget management, and promoting independence.
Conduct home visits to introduce clients to our compassionate care workers and provide ongoing support.
Utilise computer skills for efficient record-keeping and communication.

Minimum of 12+ months of experience in the Aged and Disability Sector, preferably as a care worker.
Valid QLD Driver's License.
First Aid & CPR Certificate.
Yellow Card/NDIS Worker Screening Clearance (Paid).
