The role We're looking for a Team Facilitator to support our Balcatta team.
This part-time permanent role blends direct customer support with team leadership and coordination.
You'll work closely with Support Workers and the Service Coordinator to make sure customers receive safe, consistent, and high-quality services.
This role includes a mix of: Working directly with customers in the community and their homes Providing coaching, mentoring, and supervision to Support Workers Handling rostering, service reviews, and record keeping for NDIS and other funders80% of the role is Support Worker tasks, 20% of the role is administration What you'll do You'll be supporting customers with a range of complex needs, including spinal cord injury, cerebral palsy, and Parkinson's disease.
Duties may include: Bowel care (nurse assessment) Spinal cord injury education and skin care management Catheter care and catheter flush (nurse assessment) Education on Glyceryl Trinitrate (GTN) and transdermal patches Managing autonomic dysreflexia Blood pressure monitoring Personal care, domestic assistance, mental health support Working alongside customers with challenging behaviours, using Positive Behaviour Support (PBS) strategies and training Building strong relationships with customers, their families, and the support team We'll support you with training, buddy shifts, and access to clinical guidance before you take on any complex tasks independently.
What you need A valid driver's licence and reliable vehicle NDIS Worker Screening Check (or willingness to obtain) Working with Children Check (or willingness to obtain) Current First Aid & CPR Certificate III in Disability or Aged Care (or working towards) Experience as a Support Worker, ideally with complex/high-intensity care Ability to lead, motivate, and support a team What you get from us $38.66 per hour + super + penalties + travel allowance Permanent part-time contract Mobile phone for work use HBF health cover discount (up to 8%) Employee Assistance Program A supportive and inclusive team culture Culture at Avivo We value the different life experiences, skills, and backgrounds of our diverse workforce.
