**About the role**:
The Care Management Team Leader will be a senior member of the Home Instead Melbourne Inner South and Bayside team. This role will be responsible for successfully directing and coordinating all activities associated with care management including the care of potential and existing clients.
This role provides leadership and effective management of the Care Management Team, compliance and clinical governance, and overall client experience including the delivery of quality client care services.

**Key Responsibilities**:
- Represent the business as a professional leader in the aged care sector, and as an advocate of our brand and business model within the local community.
- Provide day to day leadership for the team members to support a highly effective CAREGiver workforce and client satisfaction.
- Provide regular performance management and day to day coaching/mentoring of team members
- Review clients to understand the drivers of business growth and implement strategies that enable future growth.
- Lead and manage care management operations to ensure client care services are of a consistently high standard.
- Maintain accurate client records in the RMS, My Aged Care and the Aged Care Provider portal including new clients, discharges, assist with monthly claiming.
- Identify improvement opportunities to enhance the effectiveness and efficiency of services.
- Review and sign-off on Care Plans, ensuring that clinical needs are recorded, management plans in place and changing medical needs are recorded and appropriately responded to.
**Qualifications & Experience**:
- Registered Nurse with a bachelor's degree in nursing with AHPRA registration.
- Experience leading teams and supervising team members.
- An understanding of the Aged Care Service System and Home Care Packages
- Highly efficient and organised with proven time management skills.
- Must have great communication and attention to details skills.
- Self-motivated and outcome orientated, with the capacity to deal with multiple and at times conflicting priorities.
- Must possess a valid driver's license.
