At BaptistCare, we're looking for a dedicated Care Assessment Coordinator to join our brand-new Residential team at Glendale Lodge in Elderslie. **About the Role**:
As a Care Assessment Coordinator, you will play a vital role in coordinating the AN-ACC (Australian National Aged Care Classification) process, leading pre-admission assessments, and ensuring optimal funding outcomes. You'll work closely with our Residential Manager and Funding Consultants to support residents' care journeys and uphold the highest standards of aged care.

**About You**:
To be considered, you will have:

- A qualification as a Registered Nurse, along with experience in aged care and knowledge of the AN-ACC
- Strong interpersonal and communication skills
- Proficiency in MS Office and clinical documentation systems
- Ability to lead, train, and support staff
- Excellent time management and problem-solving skills
- Experience with ACFI and quality improvement tools - desirable
