**About us**:
The Queensland Government is committed to reforming the youth justice system to strengthen the prevention, early intervention and rehabilitation responses to youth crime in Queensland and make communities safer.
CWESH situated at the Caboolture Watchhouse is an independent campus of the Queensland Education and Training Centre (QETC). The campus will operate under a separate operational model, which will be collaboratively developed in consultation with partner agencies and key stakeholders over the coming months. Upon establishment, the hub will educate up to 31 students across a 48-week year delivery model.
QETC is a multi-campus Department of Education school operating within South-East Queensland's Youth Detention Centres at Wacol. QETC schools caters to male and female students from across the state who are either on remand or have been sentenced for criminal offences. The school plays an important role in the community by providing young people in detention with the opportunity to gain knowledge, skills, and values that can enhance their capacity to pursue positive choices in the future.

**About the Role**:
The Transition Officer is responsible for meeting placement and outcome targets by successfully engaging and supporting students to transition to appropriate pathways including education and/or employment upon release from custody. The Transition Officer collaborates with a range of stakeholders, including students and carers, Queensland Government partner agencies, and agencies from the broader service system.

**Responsibilities include (but not limited to)**:

- Establish rapport and maintain resilient relationships with students to motivate their engagement in learning, as appropriate to student's individual circumstances, to enable successful transition plan outcomes.
- Be accountable for assigned cases to progress transition plans, including undertaking appropriate case management, advocacy and support, prioritising daily caseload, communicating with transition partners and students and implementing agreed approaches.
- Establish networks with group training schemes, registered training organisations, government and non-government employment agencies, and alternative learning environments to provide employment, training and education opportunities for young people released from custody.

You will be required to work within a Queensland Police Service (QPS) custodial environment. This role involves direct contact and interaction with young people accommodated in the Caboolture Watchhouse.

**What you need to succeed**:

- The successful applicant will be able to implement strategies and programs to improve the transition outcomes of young people who may have had varied schooling experiences and a proven capacity to work effectively with them to address their future learning, training and employment pathways.

**Mandatory requirements**:

- When working in regulated employment an employee must have a current Working with Children Clearance (blue card) issued by Blue Card Services.
