**About the Role**

Our Clinical Intake Partners are responsible for the assessment, and care plan development for consumers receiving government funded Home Care packages and undertake care planning in accordance with a consumer's identified care goals and the Home Care Package framework.

We develop long-lasting relationships and are trusted partners in our consumers' home care journey. We are seeking enrolled or registered nurses who will champion this consumer-directed care approach.

**Duties**
- **Hold meetings with recipients with complex clinical care needs and/or their representatives to determine their care and support goals**:

- **Document the care plan and budget as developed with the recipient**:

- **Develop the consumer's risk profile in consideration of their medical history and targeted questioning**:

- **Make referrals via the Clinical Escalation Pathway for consumers with high impact and/or high prevalence risks**:

- **Administrate information, documentation, and systems relating to care management**

**Professional Skills**
- **Completed a qualification in Nursing & registered with AHPRA**:

- **Relevant understanding and experience in the delivery of health services**:

- **Confident and skilled in supporting and working with clients, preferably in a telehealth setting**:

- **Experience using online care management systems (desirable but not essential)**:

- **Client-focused care and case management experience (desirable but not essential)**:

- **A practical understanding of the Aged Care sector and Quality Standards (desirable but not essential)**

**Salary Package**
- **Starting salary of $80,000 per annum, plus 10.5% superannuation**:

- **Annual bonuses of up to 20% of base salary, based on service quality & efficiency**:

- **Eligible for the Aged Care Registered Nurses Payment, starting from $3,700 after 12 months of continuous employment**

**Mandatory Requirements**
- **Current Federal Police Clearance (not more than 12-months-old) and international Police Check if you have lived overseas in the last 10 years.**:
