As a new and innovative service delivery organisation based on a clear set of values, we are committed to developing and maintaining a culture that promotes; choice and control, the wellbeing and safety of the people we support, and that of our staff. Due to rapid growth, we are looking for enthusiastic and experienced Support Workers.

**The Role**

Using a person-centred and active support approach, a Support Worker will work as part of a team to provide support services to people with disabilities. The Support Worker will work with a range of plans for each client, such as an individual Support Plan, Health Care Plan, Medication Plan, Meal Plan, and Manual Handling Plan, and will work within the rhythm of service for each house.
- Maintain and monitor client support plans
- Maintain and monitor client support plans
- Ensure a client-focused and person-centred approach is maintained when engaging with a client
- Maintain professional and effective interactions with team and stakeholders
- Fulfill compliance and training requirements
- Must be prepared to work over a 24/7 roster, including mornings, evenings, weekends and sleepovers

**Skills and Essential Requirements**
- Minimum Certificate III in Disability, Individual Support or other equivalent qualification
- DHS Disability Clearance/NDIS Worker Screening Check
- Current First Aid Certificate (HLTAID003 & HLTAID001)
- Current Manual Handling Certificate
- Current Assist with Medication Certificate
- Driver's License
- Working Rights in Australia
