You will provide nursing care patients in their place of residence within the community.

Assess and document the anticipated needs of the patient.
Make relevant referrals as necessary.
Assess the occupational health and safety needs in the home and organize occupational therapy assessment to improve safety if necessary.
In accordance with the prepared nursing care plan provide care that encourages the return to independent living where appropriate.
Liaise closely with the Registered Nurse, Clinical Case Manager and treating Doctor to maintain the highest possible standard of nursing care within the home.
Observe, record and report to a Registered Nurse specific and general physical and mental conditions of patients, and symptoms and signs which may be indicative of change.Successful applicants will hold a valid State Drivers Licence and be registered with the Nursing & Midwifery Board of Australia (AHPRA) as a Registered Nurse or Enrolled Nurse.Registered Nurses must have minimum three years post graduate experience in acute nursing and have demonstrated experience in a speciality area.Above all, patient care will be at the core of everything you do committing to and supporting our Mission and Values.
