Based in West End, this mental health based non-profit has been providing education and support to clients, families, and carers for over 40 years.

As the Support Coordinator, you will be supporting participants to understand and implement the funded supports in their plans, and link them to community, mainstream and other government services, as well as supporting individuals to build skills and help connect them to providers.

**In this role, you will be responsible for**:

- Assisting participants to develop knowledge, experience, and connections with the community and broader systems of support and consider support options.
- Work with participants to understand plan funding and its purpose, and design support solutions to meet participant outcomes.
- Assist to resolve points of crisis and develop capacity and resilience in the participant's network.
- Coach the participant through challenges that may arise and assist them to prepare for plan review.
- Support Regional managers to review client plans and to ensure appropriate support is provided within a clients NDIS funding.

**To be successful, you will have.**
- Demonstrated knowledge and understanding of the NDIS.
- Relevant qualification in a human service, community sector management or a related field
- Certificate of completion of the "NDIS Worker Orientation Module" (essential)
- Must hold (or have the ability to acquire) a Yellow/Blue card and NDIS worker screening check.
- Current Police check, or the ability to get one

**A familiarity with the nonprofit and NDIS sector is very desirable.**
