**About the Role**:
Conscious Care Support is seeking dedicated **Outreach Mental Health Support Workers** to provide psychosocial, emotional, and practical support to individuals with chronic psychosocial disabilities. In this role, you'll:
? Work independently in clients' homes and the community
? Support individuals in identifying and achieving their goals
? Collaborate with care teams for high-quality, client-centred support.

This is an **outreach role**, meaning you'll work autonomously while still being part of a supportive, tight-knit team.

**What You Need**:
? **Cert IV in Mental Health (minimum)**
? Strong case note writing skills (concise & articulate)
? In-depth understanding of mental health and psychosocial disability
? A genuine passion for empowering others
? A driver's license & comprehensively insured vehicle
? A mobile phone for communication with clients and stakeholders

**Additional Requirements**:
? **NDIS Workers Screening Check**
? **Police Check (from within the last 12 months)**
? **Working with Children Check (WWCC)**

Pay: $44.00 - $77.02 per hour
