Through this achievement, we have showcased remarkable levels of patient experience, notable decreases in emergency department visits & significant cost savings for funders.For the first time in South Australia, a city hotel is proving a location for public hospital patients who no longer require acute care, to rest and recover in comfort at no cost – while freeing up beds for people who need them.This innovative model of care sees patients relocated from metropolitan and peri-urban hospital beds to the Pullman Adelaide, in their own rooms within the hotel.We are looking for a full timeSocial Workerto deliver comprehensive care for to patients with varying complexity on a full-time fixed term contract, from the 5th of September 2025 to the 7th of November 2025.Working in analternate dynamic environment, based in the Pullman Hotel, Adelaide, you will utilise innovative technology and be rewarded in a role where your expertise will contribute to shaping exceptional health outcome for sub-acute patients.Here's a glimpse of what you will be doing:Collaborate with a multidisciplinary team to develop and implement integrated service models and clinical protocols.Facilitate group sessions and contribute to health promotion activities as required.Provide effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow up.Advocate for patient's needs and coordinate access to servicesParticipate actively in clinical review meetings and continuous professional development.About youYou are passionate about the provision of innovative models of care and excited by this dynamic way of working.
Additionally, you have:Degree in Social Work, with eligibility for membership with the Australian Association of Social Workers (AASW).
Demonstrated experience in a sub-acute or similar healthcare setting.Strong communication, advocacy, and problem-solving skills.Excellent communication and interpersonal skills, with the ability to adapt to evolving service need.Proficiency in using Microsoft Office suite and other relevant documentation systems.Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
