**ABOUT US**

Connecting Care Support Services is a Disability Support Provider servicing NDIS participants in the South East Queensland Region including Moreton Bay, North Brisbane, Central Brisbane, South Brisbane, Logan, Ipswich and surrounding areas.

We provide In Home Supports, Personal Care and Community Access while using a person-centred approach to support our participants towards achieving their goals.

We are looking for staff who are naturally positive, proactive, caring and adaptable. You must have great communication skills and be enthusiastic about making a positive difference in people's lives.

**ABOUT THE ROLE**

Disability Support Workers assist NDIS participants towards achieving independence and a better quality of life. Support Workers may be required to work in the clients' homes and provide both physical assistance and emotional support. Support may also be provided in clinics, hospitals and out within the community.

Day to day tasks as a Disability Support Worker may include:

- Assisting people with important daily activities, booking appointments, budgeting, goal setting.
- Performing Domestic Assistance such as food preparation, cooking, Housework/ Cleaning.
- Undertaking tasks outside the home such as shopping, transport to appointments, support at the gym.
- Ensuring a varied program of social activities and access to social services.
- Providing emotional support, professional friendship and guidance on decision making.
- Facilitating daily personal hygiene, showering and dressing tasks (depending on the client needs).

**ABOUT YOU**

To be considered for this role, you will need the following, or be willing and able to obtain:

- Own reliable vehicle and Driver's License Including Full Comprehensive Car Insurance (willing to obtain)
- Certificate III in Individual Support/ or similar (within the first 6-months of employment)
- Current First Aid & CPR (willing to obtain)
- Police Check (willing to obtain)
- NDIS Yellow Card - Yellow Card (willing to obtain)
- Working With Children Check - Blue Card (willing to obtain)

Licence/Certification:

- First Aid Certification (preferred)
- Working with Children Check (preferred)
- NDIS Worker Screening Check (preferred)
- National Police Certificate (preferred)
- Driver Licence (preferred)

**Job Type**: Casual

**Salary**: $30.00 - $57.66 per hour

Ability to commute/relocate:

- Mount Gravatt, QLD: Reliably commute or planning to relocate before starting work (required)

Licence/Certification:

- First Aid Certification (preferred)
- Working with Children Check (preferred)
- NDIS Worker Screening Check (preferred)
- National Police Certificate (preferred)
