We are seeking a qualified and experienced Support Coordinator to join our growing team.

In this role, you will work closely with NDIS participants to coordinate their supports, build independence, and ensure they are connected with the right services to achieve their goals.

Key responsibilities include:

  • Assisting participants to understand and implement their NDIS plans
  • Coordinating and monitoring supports to ensure quality and effectiveness
  • Building strong relationships with participants, families, and service providers
  • Maintaining accurate records and progress notes
  • Identifying barriers, managing risks, and developing practical solutions
  • Promoting participant choice, control, and self-advocacy

About You

You are a compassionate, organised professional who takes pride in making a difference. You understand the NDIS and thrive in a role where each day brings new opportunities to empower others.

Essential requirements:

  • Certificate IV (or higher) in Community Services, Disability, Mental Health, Social Work, or related field
  • Previous experience in Support Coordination, Case Management, or Community Services
  • Sound knowledge of the NDIS framework and Practice Standards
  • Excellent communication and organisational skills
  • Strong computer and administrative skills
  • Current Driver's Licence and reliable vehicle
  • All required checks (NDIS Worker Screening Check, Police Check, and Working With Children Check) must be current and valid at the time of application
