About the role
Permanent full time opportunity to provide high level Clinical Pharmacy services and leadership on a rotational basis to multiple speciality Pharmacy work units and multidisciplinary teams and to stakeholders/customers of Queensland Health in accordance with organisational goals.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide and be individually accountable for the delivery of high level Clinical Pharmacy services (including but not limited to medication history taking, medication reconciliation, development of Medication Action Plans, pharmaceutical care plans, and therapeutic drug monitoring) to multi-speciality work units and multidisciplinary teams with minimal direct supervision.
- Provide clinical advice and assistance to line management, relevant service managers and other members of the multidisciplinary team as required regarding the management of clinical work practices, service delivery, quality assurance and the prioritisation and development of complex clinical services as it relates to their area of responsibility.
- Actively contribute to the initiation, development, implementation and monitoring of clinical practices, professional standards, protocols, education, collection of key performance indicator data and quality improvement activities within multiple speciality Pharmacy work units and multidisciplinary teams as well as monitoring and reporting on outcomes of the changes to service delivery.
