- Relevant qualification in Psychology, Social Work or other related discipline – post graduate qualifications preferred
About the role
An exciting opportunity to participate in a new program and team at QuIHN.
Part time - 30hrs
Fixed Term to 26/06/2026
Location - Capalaba
The Case Manager works with clients accessing QuIHN Therapeutic Services, and their counsellors in a case management capacity, including travelling to identified outreach areas.
Key responsibilities include:
- Provide an outreach case management program and implement case plans
- Conduct home visits within outreach areas in accordance with established policy and procedures
- Support clients through the stages of change process with relapse prevention goals and/or harm minimisation goals
- Support clients, and significant others, with welfare needs, housing, education and employment, through active referral pathways as part of a case management plan
- Deliver brief interventions and/or facilitate the provision of information, psycho-education and crisis support
- Conduct intake work through initial screens and ongoing case management planning, including conducting risk assessments with clients
What we're looking for
Someone who is passionate about their work and the health outcomes of both clients and the broader community, and can demonstrate:
- Qualification of Certificate IV level or higher in a related field.
- Ability to engage and work with vulnerable and complex individuals and/or families affected by substance use and other related concerns
- You are an ally of the AOD community or have lived or living experience of substance use, as well as experience working with marginalised and disadvantaged populations
- Intermediate competency in the use of Microsoft packages
- Available to work between the hours of 8:30am - 7:00pm, Monday-Friday
- Current Queensland driver's licence (or equivalent)
