Care Advisor

About the role

Support Network is seeking a dedicated and compassionate Care Advisor - Home Care Package to join our small friendly team.  This casual role will see you predominately working remotely while providing support and guidance to our Home Care Package clients . Your role will be instrumental in ensuring our clients receive the care and services they need to live at home independently for longer.

What you'll be doing

• Undertake comprehensive assessments of clients in collaboration with the client and their family
• Determine health & wellbeing maintenance and promotion strategies aimed at encouraging independence and assisting the client to maintain quality of life
• Regularly monitor and review client care plans to ensure they continue to meet their evolving needs
• Identifying and organising support worker to provide the agreed client services
• Ensuring all equipment and/or other agreed resources are purchased or arranged for the client
• Maintain detailed client records and documentation in accordance with industry standards

What we're looking for

• Minimum 2 years of experience in a similar care advisory or case management role within the Aged & Disability Support sector
• Relevant tertiary qualification
• Strong understanding of the Home Care Package program and related government regulations
• Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families
• Proactive and self motivated, possessing a strong work ethic and the capability to work independently and collaboratively within a team.
• Sydney based candidate - although this role is predominately work from home you will, on occasion, be required to attend face to face meetings at short notice. Only Sydney based candidates will be considered for this role.
