Nurse Next Door is searching for a dynamic Care Designer (Registered Nurse) to ensure our clients experience safety, comfort, and happiness in their homes. You'll also lead a dedicated team of caregivers and nurses who share your passion for delivering exceptional care. This role is also known as Care Manager or Clinical Team Lead in other organisations.

In this leadership role, you will ensure our clients receive the highest standard of care in the comfort of their own homes. Reporting directly to the Clinical Care Director and working in collaboration with an exceptional leadership team, you will be responsible for business development, clinical support, and overseeing care for current clients. You will manage a growing team of Care Ambassadors as well as our Community Caregivers and Nurses.
This is an excellent opportunity for a senior clinician or current Clinical Manager seeking a long-term, stable role within a locally owned business that genuinely values its people.
Key responsibilities include:
- Advocating for Making Lives Better and Happier Ageing by helping clients live independently and do what they love
- Lead the Care Team to ensure the highest standards of care and compliance with NDIS and Aged Care Quality Standards.
- Manage clinical documentation, assessments, care plans and audits.
- Provide mentorship, coaching and performance management of clinical staff.
- Collaborate with the Clinical Care Director and Managing Director to support the overall operational and strategic goals of the business.
- Be accountable for upholding the high clinical standards set by Nurse Next Door

We're looking for someone who:
- Has a minimum of 5 years of experience as a Clinical Lead/Manager in a community environment (AHPRA Registered Nurse)
- Is apassionate clinical leader who is committed to delivering high-quality, person-centred care and has a passion for making a positive difference in the lives of others
- Has astrong clinical knowledge and experience in complex care, catheterisation, medication management, diabetes management, clinical governance and compliance in line with NDIS and/or Aged Care Quality Standards.
- Thrives in leadership roles, with the ability to mentor, empower, motivate and support a multidisciplinary team to achieve excellence in care delivery and a track record of meeting KPIs.
- Is solutions-focused and proactive, with strong decision-making skills and the ability to navigate challenges with empathy and professionalism.
- Values meaningful relationshipsand approach interactions with clients, families, staff and vendors with warmth and respect.
- Demonstrates a growth mindsetand commitment to ongoing learning
- Has strong organisational, communication (written and verbal), interpersonal and time-management skills
- Proficiency with technology including mobile apps, desktop applications and Google Suite
- Experience in community health, disability nursing or in-home care is highly desirable

Training/Licence Requirements:
· AHPRA Registration as a Registered Nurse (with no restrictions)
· Current CPR & First Aid Certification
· Current Australian Drivers License (with no restrictions)
· National Police Check (Aged Care/ Vulnerable Persons Sector)
· NDIS Worker Screening Check
· NDIS Worker Orientation Module Completion
· ACT Working with Vulnerable People card
