Position: Part Time (3-4 days per week)

Location: Outer South Eastern Suburbs (Dandenong/ Berwick / Cranbourne/ Pakenham areas)

The Role

As a Community Physiotherapist with Agestrong, you will:

• Provide assessment, treatment, and rehabilitation for patients post hospitalisation in their homes.
• Work with older adults to maintain or improve function, balance, strength and help improve/ maintain their quality of life.
• Design and deliver home‑based physiotherapy programs, which may include falls prevention, gait retraining, assistive device prescription & training, and safe mobility practice.
• Collaborate with other allied health professionals, hospital discharge planners, aged care providers, and community services.
• Maintain clear documentation, outcome measurement, and reporting.
• Travel locally between client homes.

Who We're Looking For

You are someone who:

• Has a degree in Physiotherapy, and hold general registered with AHPRA.
• May be a new graduate (we welcome and support new grads) or experienced clinician.
• Demonstrates excellent interpersonal skills, empathy, and an ability to build rapport with older adults.
• Has strong clinical reasoning and ability to work independently in a community/home setting.
• Is organised, reliable, and comfortable managing travel between clients.
• Holds a valid driver's licence and access to a vehicle.
• Is committed to continuing professional development and evidence‑based practice.
