About Agestrong Health Group

Agestrong Health Group has been delivering physiotherapy services in the community for over a decade, with a strong emphasis on evidence-based practice, person‑centred care, and improving quality of life for all Australians.
Position: Part Time (3-4 days per week)
Location: Outer South Eastern Suburbs (Dandenong/ Berwick / Cranbourne/ Pakenham areas)
The Role
As a Community Physiotherapist with Agestrong, you will:
- Provide assessment, treatment, and rehabilitation for patients post hospitalisation in their homes.
- Work with older adults to maintain or improve function, balance, strength and help improve/ maintain their quality of life.
- Design and deliver home‑based physiotherapy programs, which may include falls prevention, gait retraining, assistive device prescription & training, and safe mobility practice.
- Collaborate with other allied health professionals, hospital discharge planners, aged care providers, and community services.
- Maintain clear documentation, outcome measurement, and reporting.
- Travel locally between client homes.
Who We're Looking For
You are someone who:
- Has a degree in Physiotherapy, and hold general registered with AHPRA.
- May be a new graduate (we welcome and support new grads) or experienced clinician.
- Demonstrates excellent interpersonal skills, empathy, and an ability to build rapport with older adults.
- Has strong clinical reasoning and ability to work independently in a community/home setting.
- Is organised, reliable, and comfortable managing travel between clients.
- Holds a valid driver's licence and access to a vehicle.
- Is committed to continuing professional development and evidence‑based practice.
