The Role
The Aftercare service is a key aspect of the Culture Care Connect (CCC) Suicide Prevention Program, which works in collaboration with existing regional mental health services and the SEWB Program within Wathaurong.
The purpose of the CCC Aftercare program is to:
- Provide aftercare services to Aboriginal and Torres Strait Islander individuals who have been referred following a suicide attempt or suicidal crisis in line with agreed service delivery model.
- Improve Aboriginal and Torres Strait Islander Mental Health and Social & Emotional Wellbeing
- Provide culturally appropriate support and treatment for community members experiencing mild to moderate mental health challenges and significant risk of suicide.
- Prioritise the wellbeing, safety, and stability of the Community member.
- See the Community member's best interests through a Cultural lens.
- See Community members within the context of their past and present experiences.
- Empower Community members to make important decisions about themselves.
- Use trauma-informed and recovery orientated practices.
- Respect and promote connection to family and community.
- Assist Community to access both Wathaurong and external services and help with service navigation.
About you
- Qualification in Social Work, Psychology, Nursing and or Aboriginal and/or Torres Strait Islander Primary Health Care.
- Demonstrated experience working with Aboriginal and Torres Strait Islander communities in a health, social, or drug and alcohol service delivery environment.
- Excellent written and oral communication skills.
- Demonstrated experience in trauma informed practices, client-centered approaches, family inclusiveness, and strengths-based approaches to work with community.
- Client focused with an ability to advocate on the client's behalf.
- Ability to multi-task, prioritise workloads and meet deadlines.
- Willingness to undertake further study as required. Ability to work independently and in a team environment.
- Commitment to personal and professional development including the sharing of knowledge, information and skills with work colleagues
- Knowledge of safe and appropriate culturally centred practices for Aboriginal and/or Torres Strait Islander focused services, including employment of Aboriginal and/or Torres Strait Islander people.
- Demonstrated understanding of social, physical, economical, and cultural strengths and challenges affecting Aboriginal communities and their ability to access adequate and quality services.
- Understanding of the purpose and expectations of the Victorian Child Safe Standards and demonstrated commitment to contributing to a child safe organisation in both practice and culture.
Desirable
- Identify as Aboriginal and/or Torres Strait Islander.
- Demonstrated experience working with Aboriginal Community Controlled Organisations.
