About ECHO
ECHO Community Services is a registered charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence.
We provide in-home services 7 days per week for Home Care Package and 5 days per week for CHSP.
Our goal is to provide services at the same time, on the same day and with the same team of Support Workers which means that you know the people who are coming and the time they will arrive. Having the same support workers providing services, in particular, medication prompts and personal care, maintains the person's dignity and well-being. You know and can trust the people attending. There is continuity of care for people who receive our services.
Description:
The Role:
You will play a key role in providing direct patient care by closely working with the clinical team. Your duties will include but not be limited to:
Delivering clinical and other care needs in a safe, respectful and compassionate manner.
Ensuring compliance with aged care quality standards, legislation, and professional practice requirements.
Reporting and documenting any changes or concerns affecting clients' wellbeing.
Supporting clients to achieve independence and make choices in line with their individualised care plans.
Managing or contributing to specialist clinical portfolios such as: wound care, medication management, pain management, infection control, continence, nutrition, palliative care, falls prevention, and dementia support.
Assisting with data collection and analysis to support continuous improvement in care outcomes.
Applying clinical care indicators to monitor and enhance client wellbeing.
Hours: Monday to Friday, 8:00 am to 4:00 pm. You must also be willing to participate in a flexible roster involving extended hours.

Qualifications:
Qualified Enrolled Nurse with current AHPRA registration.
Skills and Experience
Essential:
Registration with AHPRA as an enrolled nurse.
Demonstrated experience in providing clinical assessments, wound care, medication administration, palliative and dementia care.
Demonstrated proficiency in Microsoft Office Suite applications.
Good understanding of continuous quality improvement, quality standards, and accreditation processes.
Desirable:
Experience in residential aged care preferred - new graduates with prior experience as a support worker are also welcome to apply.
Competencies:
Demonstrated person-centred communication skills and behaviours, particularly with clients with dementia and/or high clinical needs.
Proven ability to assess, plan, implement, and evaluate client care in consultation with a multidisciplinary team.
Above average analytical and problem-solving skills, with the ability to prioritize tasks, work under pressure, meet deadlines, and use resources efficiently.
Above average interpersonal skills and the ability to communicate with diverse backgrounds and experiences.
Attention to detail.
Client and customer focus.
Other:
Current First Aid & CPR certificates.
National Police Check and valid WA Driver's Licence with access to a reliable vehicle.
