We are seeking experienced Care Managers and Clinical Care Consultants to join our team on a contract basis. This is a unique opportunity for professionals who want the flexibility of contract work while combining their clinical expertise with travel across diverse regions of Australia—supporting providers, mentoring teams, and directly improving the lives of older Australians.

Role Responsibilities:

  • Provide hands-on coaching and leadership to RN's, EN's and Care Staff.
  • Lead person-centred care planning and evaluation for Residents/Individuals - including case conferencing with families and significant others.
  • Evaluate residents' comments, complaints and clinical incidents.
  • Assess and implement strategies to mitigate clinical risks while overseeing medication and wound management.
  • Formulate and implement strategies in partnership with our clients to meet and exceed regulatory requirements, in line with the Strengthened Aged Care Standards.
  • Evaluate all clinical risk to residents, and direct consultations with Medical and Allied health as required.
  • Recommend and implement targeted education to staff, fostering continuous improvement.

What We're Looking For :

  • Current AHPRA registration.
  • Registered Nurse (RN) AHPRA registration is required.
  • Recent experience in aged care clinical management.
  • Minimum 2 years of aged care clinical management experience is required.
  • Strong clinical assessment and care planning skills.
  • Advanced leadership capabilities with proven governance and oversight experience.
  • Excellent interpersonal and communication skills.
  • A proactive, dynamic approach with a commitment to quality and safety.
  • Police Record Check/NDIS clearance (or willingness to obtain).
  • Ability to meet Key Personnel requirements.
