As our Case Manager you will provide critical support to vulnerable families helping them keep their children safely at home. You will conduct initial assessments of each family's strengths and needs, develop case plans that empower them to create lasting change, strengthen their resilience to reduce the risk of significant harm, and connect them with appropriate services and networks.

  • Provide intensive home visiting (minimum once per week) and practical, evidence-informed interventions.
  • Maintaining accurate, timely case notes and documentation to meet compliance and reporting standards.
  • Collaborate with other services to ensure families are connected to the right supports.
  • Partner with parents and carers to build safety, stability, and resilience within the home.
