Health Care Navigator
to join our team in
Wagga Wagga (Wiradjuri Country)
on a
part time
(30 hours per week) basis In this role you will collaborate with our care coordinators in addressing the diverse social needs that influence health outcomes. You will take a holistic approach, helping individuals overcome barriers related to finances, transportation, housing, legal issues, and social isolation. Selection Criteria: The Essentials for Our Health Care Navigator role –

• Qualifications that Shine: You hold recognised credentials in Community Services, Human Services, Social Work, or a Health-related field. You've got the knowledge to back up your passion
• Advocacy Champion: You've got hands-on experience advocating for and connecting participants with local services (aged care, NDIS, community services) to help them hit their health and wellbeing goals. You're their go-to resource
• Person-Centred Pro: You know how to put the "person" in person-centred care You excel at working collaboratively with participants and their families to create tailored support plans.
• Health Detective: You have a keen understanding of the wider determinants of health—social, economic, and environmental factors—and how they impact communities. You see the big picture
• Relationship Builder: You're a networking ninja You thrive on collaboration, forging strong partnerships across diverse organisations to deliver the best outcomes.
• Communication Pro: Your communication skills are top-notch You connect effortlessly with people from culturally and linguistically diverse backgrounds and First Nations communities.
• Tech-Savvy: You're comfortable with computers and adapt easily to new software applications. You embrace technology to enhance your work
