This role provides assessments to establish goals specific to the client's needs to develop appropriate strategies and actions

The Role

An opportunity has become available for a Home Support Assessor (Aged Care Assessment Team) to join Gold Coast Health (GCH) where the focus of our strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

  • Provide support services to determine the priority needs of senior Queenslanders
  • Develop a client centred Support Plan with the client and /or representative
  • Review Reablement Focused Programs within appropriate timeframes
  • Undertake home support, face to face simple assessments

About you

To be successful in this position you will require:

  • Certificate III in Home Care or Aged Care (or equivalent work experience)
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Driver Licence
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

  • Temporary full time (up to 30 June 2026, with the possibility of extension)
  • Base salary ranging from $ $ per fortnight

The Single Aged Care Assessment System is a multi-disciplinary team providing assessment services to older Queenslanders in the community. The assessment system aims to simplify and improve access to aged care services. The assessment framework is designed to streamline the assessment process and enhance the overall experience for our older Queenslanders.
