Our Core Focus: Chronic Disease Management

At Coastal Lakes, chronic disease management is central to our model of care. We take a proactive, team-based approach to managing conditions such as diabetes, cardiovascular disease, asthma, and mental health. Our experienced nurses play a key role in delivering health assessments, care plans, and reviews, allowing GPs to focus on strategic and personalised care.

We've invested heavily in streamlining CDM workflows and patient engagement strategies, and continue to enhance our systems to support optimal patient outcomes and GP remuneration. You'll have the tools, time, and team support to deliver high-quality chronic disease care without the administrative burden.

What You'll Be Doing:

• Managing a mix of general practice consultations and acute presentations in our urgent care clinic
• Delivering comprehensive chronic disease management plans and preventative health services
• Collaborating with a multidisciplinary team to ensure high-quality, coordinated care
• Participating in continuous improvement initiatives to optimise patient outcomes

What You'll Need:

• FRACGP or equivalent
• Current AHPRA registration
• Medicare provider number (or eligibility to obtain one)
• DPA/DWS eligibility
• Experience in urgent care not essential
