About the role

As an Outreach Worker, you'll collaborate with Brimbank City Council and community-led organisations to deliver both outreach and onsite services. You'll be supported by the Brimbank Local Practice Manager and maintain connections with cohealth's Homelessness and AOD programs.

Key responsibilities include:

  • Conduct assertive outreach across key locations to engage people experiencing homelessness and/or substance use
  • Build therapeutic relationships, assess needs, and make warm referrals
  • Coordinate care through person-centred planning, advocacy, and support
  • Navigate service barriers such as stigma, literacy, or lack of awareness

Our ideal candidate:

  • Relevant tertiary qualification and/or significant experience in housing, AOD, mental health, disability, or welfare
  • Strong understanding of homelessness and complex client needs
  • A current Victorian Driver's License
