About the role
BRAMS is seeking a Support Coordinator to join our Community Services Team. The role works alongside Aboriginal and Torres Strait Islander participants and their families to implement NDIS plans, connect with supports, and build independence.
Key responsibilities include, but are not limited to:
- Support participants to understand and use their NDIS plans
- Coordinate NDIS, mainstream, and community supports
- Build informal networks and improve quality of life
- Prepare reports and maintain accurate records
- Deliver services that reflect BRAMS' Model of Care, Quality Care Framework, and Consumer Voice Framework
About you
To be considered for this role, please submit a cover letter and resume. Your cover letter must address the selection criteria below:
- Qualifications in Community Services, Disability, Mental Health or Allied Health
- At least two years' experience in disability, community services, or allied health
- Strong knowledge of the NDIS and culturally safe, person-centred practice
- High-level communication and organisational skills
- Current C Class Driver's Licence required
