About the Role

Sydney Local Health District's Community Health Centres have a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.

"Community Mental Health Team" (Core Team) is the mainstay of the community mental health system. The Core Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination.

Ideal Candidate

  • Current NSW Drivers Licence (P1 and P2 acceptable).
  • Recognised tertiary qualifications appropriate to profession. Full registration and eligibility for membership to professional association, as appropriate.
  • Demonstrated competencies to provide contemporary mental health practice.
