ABOUT THE ROLE
Due to our continued growth, we have an exciting opportunity for a motivated, self-directed, and experienced Clinician to join our team as a Care Manager, reporting directly to a Regional Care Manager.
- A Registered Nurse by profession, the Care Manager is a key role responsible for the oversight of care and services to clients
- This rewarding and hands on role requires a proactive approach to the planning, coordination, delivery, and care management of clients and services.
- Working closely with the Rostering Team, Client Liaison Team and the role includes engaging with and leading our care team, consisting of Nurses and Carers.
- Full-time position available in Mornington
We are looking for people who have:
- A current registration with AHPRA as either a Division 1 Registered Nurse
- Registered Nurses with at least 5 years experience in the field.
- A passion for caring for people and delivering a high standard of care.
- Excellent communication, customer service and interpersonal skills.
- Exceptional organisational and problem-solving skills.
- Access to a smartphone.
- A current driver's license and access to a registered, insured and well-presented car.
- The right to work in Australia & can obtain a satisfactory Police Check.(Mandatory)
- NDIS Worker Screening Check (Mandatory)
